Very, very comprehensive, to-the-point article!
I have some slightly different approaches for two factors and a what I think it to be, good and easy method to further improve your site's overall buoyancy inthe SERPs.

1) concerning the meta description, namely I use my targeted keyword in this meta tag because Google will display it in the SERPs ONLY if it contains the keyword searched for; and a well written title backed by a compelling meta description increases click-thru rate;

2) I don't use the keyword in headers h1, h2, h3... because Google perceives this as a spammy practice;

One last point I want to mention has to do with the way content is structured within the site. This SEO method, better know as Silo Structuring, retrieves a big chunck of the market share that on page optim. lost in favour to off page optim, over the last years.

If you care to know more about silo structuring your website, here's a post I've written on the topic:

Hope you'll find it useful!

Thanks for this post. I have definitely heard that using your keyword phrase in the meta tags as well as on page (H1) tags has a great influence on where your page is ranked. I've also read that by having your keyword density between 6 - 8% is ideal.

In my own opinion there are many SEO tweeks that can be done, however nothing beats stong inbound links and good content, that is added to regularly

And while we are on the subject of keyphrases and SEO, let's not overlook their influence when present in the URL or domain name.
